A glass downstem pinches the bong's water pool at an angle higher than the water pool to increase the bubbling inside the bong. There are different types of stems such as six arms or one arm bong stems. In general the more diffuser slits a bong stem has the more bubbles will happen inside the bong thus more crisper hits you will get.

A stem can be connected to other stems (ash catchers) and then to the water pipe for more efficiency.
